A FULL TREATMENT FEE WILL BE CHARGED WITHIN 24 HOURS NOTICE OF CANCELLATION IS GIVEN.

A FULL TREATMENT FEE WILL BE CHARGED FOR FAILURE TO ATTEND WITHOUT NOTICE.

I understand that my contract for physiotherapy treatment is with The Partnership and that I am responsible for all payments. This applies to Private Medical Insurance also should they not pay.

Payment is requested following each treatment; unless claiming through Private Medical Insurance. If you have an excess on your policy you are requested to pay this directly to Paul Massey associates.
